— 1) Application must be completed by you and your qualifying condition certified by an

ARIZONA PROFESSIONAL. See part C. (enclosed)

— 2) Conditions of Acceptance form. (enclosed)

— 3) LEGIBLE COPY of state issued ID or utility bill in applicant’s name to prove ARIZONA

residency.

— 4) As of January 1, 2009 proof of Legal United States residency is required which can be proven

with the following documents for U.S born citizens:

a) COPY of U.S. PASSPORT with current name, OR

b) COPY of BIRTH CERTIFICATE also accepted, however if the name appears different,

a COPY of the supporting name change documentation such as marriage certificate(s), etc. must

be submitted to prove identity

For all other legal residency situations, please see long form.

— 5) Arizona Statement of Citizenship and Alien Status for State Public

benefits form to accompany above document(s). (enclosed)

— 6) Please read catalog and mark the preference sheet. (enclosed)
